  • If you have never been seen at one of our facilities, please call the Appointment Center for your location to register as a new patient. Ohio | Florida
  • Indian River Patients: If you have not had an appointment since Sept. 20, 2020, please call 772.410.0560 to update your registration.
  • If you have previously opted out of including your social security number in your record, you may have issues signing up or resetting your password. If this is the case, please call the Appointment Center for your location.
  • Martin Health Patients: Log in to your MyChart here.
